<h2 style="text-align: justify;"><strong>Baclofène : le médicament qui guérit de l&rsquo;alcoolisme ?</strong></h2>
<p style="text-align: justify;"><strong><span style="color: #999999;">Baclofène, une vérité qui dérange. On pourrait ouvrir l&rsquo;information avec ce titre tapageur. Malheureusement, l&rsquo;expression est déjà utilisée et l&rsquo;ampleur médiatique du film d&rsquo;Al Gore n&rsquo;atteint pas celle du fameux médicament. Car depuis quelques jours, les choses bougent autour du baclofène. </span></strong></p>
<p style="text-align: justify;"><strong><span style="color: #999999;">Mais pour comprendre l&rsquo;histoire du médicament, il est des histoires humaines qu&rsquo;il convient de se remémorer. Tout commence en 2004. </span></strong></p>
<p style="text-align: justify;"><strong><span style="color: #999999;">Olivier Ameisen, brillant cardiologue, découvre qu&rsquo;un médicament, le baclofène, à forte dose, peut guérir l&rsquo;alcoolisme. Utilisé à l&rsquo;origine pour traiter les défaillances musculaires de malades atteints de la sclérose en plaques, le médicament se révèle également efficace contre ce qu&rsquo;on appelle désormais le craving. Ce mot anglais désigne, au delà de l&rsquo;aspect d&rsquo;addiction et de maladie, l&rsquo;envie irrépressible de consommation d&rsquo;un produit psychoactif. Olivier Ameisen décide donc de publier ses conclusions à partir de décembre 2004 dans plusieurs revues scientifiques, notamment américaines. Le cardiologues français demande la réalisation d&rsquo;essais cliniques, pour confirmer ou infirmer son modèle thérapeutique. A son grand dam, les autorités françaises ne lancent aucun essai.</span></strong></p>
<p style="text-align: justify;"><strong><span style="color: #999999;">C&rsquo;est la sortie de son livre qui va enfin permettre à Olivier Ameisen de mettre en lumière son histoire, ainsi que sa récente découverte. « Le dernier verre », sorti en 2008, rencontre un succés médiatique. Mais rien n&rsquo;avance du côté des essais cliniques. Des personnes vont pourtant se battre pour que les choses évoluent. Parmi eux Yves Brasey.</span></strong></p>

But a small group of physicians now prescribe it to patients suffering from alchoholism as a way to reduce patients’ craving for alchohol. Rehman Tungekar spoke with doctors and patients using the medication to help kick the habit. Rebecca agreed to meet in a quiet park, where there would be few people to overhear her speaking. Her real name isn’t Rebecca – she asked for an alias to be used. Rebecca’s been drinking since she was a teenager, but it was only in the past two years that she really began drinking excessively. “Oftentimes I would black out,” she recounted, “and every time I did it I would tell myself ‘Well, I’m not going to do that tomorrow night.’ And yet I did.”</strong></span></p>
<p style="text-align: justify;"><span style="color: #808080;"><strong>In the past, Rebecca had tried Alcoholics Anonymous and even saw an addiction specialist, but it never seemed to be enough. “I think my drinking improved for a bit, but then it just kind of went back to drinking excessively,” she said, “so I really decided to start researching pharmacological agents to help with alcoholism.”</strong></span></p>

<p style="text-align: justify;"><strong><span style="color: #888888;">The Hotel Lutetia is a beautiful belle époque building in Paris&rsquo;s sixth arrondissement. It&rsquo;s a place steeped in history: Josephine Baker was a resident, and it was here that General de Gaulle spent his wedding night. It was also here, on 26 January 2000, that Dr Olivier Ameisen, first official physician to the prime minister of France under Raymond Barre, noted cardiologist at Cornell University, talented pianist and friend of both Nobel Peace Prize-winner Elie Wiesel and record producer Arif Mardin, received the Légion d&rsquo;Honneur for his « contribution to the image of France abroad and to cardiology ».</span></strong><br />
<strong><span style="color: #888888;">A proud moment in a life of excellence and achievement, you would imagine, but you&rsquo;d be wrong. Sitting in the bar of the Lutetia 10 years later, Ameisen, now 56, recalls how he felt: « When Barre and all those guys were kissing my cheeks, I thought: &lsquo;Where are their brains?&rsquo; I mean, when I was accepted at Cornell I looked at those guys and I thought that they were mediocre – that if those guys want me, they are idiots. »</span></strong></p>

<p style="text-align: justify;"><strong><span style="color: #888888;">In the treatment of alcohol dependence, Marc Schuckit (Feb 7, p 492)1 omits baclofen. This is most regrettable for two reasons. First, Schuckit’s proposed treatment excludes patients with liver cirrhosis from receiving any anticraving medication. Alcohol intake in patients with cirrhosis is associated with high mortality.2 With cirrhosis, naltrexone is contraindicated and the safety of acamprosate, topiramate, and disulfi ram has not been tested because these agents undergo extensive liver metabolism. Baclofen is the only anticraving medication shown in a randomised trial3 to be safe and</span></strong> <strong><span style="color: #888888;">eff ective at promoting abstinence in patients with cirrhosis.  Depriving</span></strong> <strong><span style="color: #888888;">cirrhosis patients of baclofen would deprive them of the benefit patients</span></strong> <strong><span style="color: #888888;">without cirrhosis get from standard anticraving treatment.</span></strong></p>

<p style="text-align: justify;"><strong><span style="color: #888888;"><span style="text-decoration: underline;">Aims:</span> To further test whether the baclofen-induced suppression of motivation to consume alcohol in animals could be</span></strong> <strong><span style="color: #888888;">transposed to humans.</span></strong></p>
<p style="text-align: justify;"><strong><span style="color: #888888;"><span style="text-decoration: underline;">Methods:</span> A patient who had neither tolerated nor benefited from other alcohol treatment modalities was put on trial with baclofen on a dosage up to 140 mg/day.</span></strong></p>
<p style="text-align: justify;"><strong><span style="color: #888888;"><span style="text-decoration: underline;">Results:</span> The patient reported dramatic reduction in cravings for and preoccupation with alcohol.<br />
<span style="text-decoration: underline;">Conclusions:</span> High-dose baclofen therapy was associated with complete and prolonged suppression of symptoms and consequences of alcohol-dependence.</span></strong></p>

<strong><span style="color: #888888;"> Cette image provient de cerveau de cocaïnomanes sevrés et à qui on montre un indice de prise de cocaïne, par exemple une ligne blanche, ou quelque chose qui rappelle la drogue et qui déclenche chez eux l’envie d’en reprendre. C’est une chose très connue, c’est un modèle utilisé universellement par l’imagerie cérébrale, c’est très intéressant d’ailleurs.</span></strong><br />
<strong><span style="color: #888888;"> Quand on montre à quelqu’un qui a été sevré en cocaïne un indice qui lui donne envie d’en prendre, on voit s’allumer son cerveau : toutes sortes de structures impliquées dans la dépendance aux drogues et qui s’appellent l’amygdale, l’insula, le striatum, deviennent hyperactives quand la personne a envie, quand elle a du craving.</span></strong><br />
<strong><span style="color: #888888;"> L’image allumée de gauche est l’image du craving d’un cerveau qui a envie de drogue.</span></strong><br />
