<p style="text-align: justify;"><span style="color: #888888;"><strong>Des données cliniques et des témoignages de patients qui indiquent une efficacité du baclofène dans le traitement de l’alcoolisme se sont multipliés au cours de ces deux dernières années, mais beaucoup de prescripteurs sont toujours très réticents à l’utiliser. Pourtant, l’alcoolisme est une maladie mortelle pour laquelle n’existe aucun traitement véritablement efficace, en dehors du baclofène. Dans cet article, l’auteur rapporte son expérience de prescripteur de baclofène, avec une analyse des obstacles potentiels susceptibles de limiter son efficacité. Une approche des mécanismes biologiques par lesquels pourrait agir le baclofène est aussi proposée.</strong></span></p>

<p style="text-align: justify;"><strong><span style="color: #888888;">In the treatment of alcohol dependence, Marc Schuckit (Feb 7, p 492)1 omits baclofen. This is most regrettable for two reasons. First, Schuckit’s proposed treatment excludes patients with liver cirrhosis from receiving any anticraving medication. Alcohol intake in patients with cirrhosis is associated with high mortality.2 With cirrhosis, naltrexone is contraindicated and the safety of acamprosate, topiramate, and disulfi ram has not been tested because these agents undergo extensive liver metabolism. Baclofen is the only anticraving medication shown in a randomised trial3 to be safe and</span></strong> <strong><span style="color: #888888;">eff ective at promoting abstinence in patients with cirrhosis.  Depriving</span></strong> <strong><span style="color: #888888;">cirrhosis patients of baclofen would deprive them of the benefit patients</span></strong> <strong><span style="color: #888888;">without cirrhosis get from standard anticraving treatment.</span></strong></p>

<p style="text-align: justify;"><strong><span style="color: #888888;">I wish to propose a hypothesis that could help explain some</span></strong> <strong><span style="color: #888888;">of the effects of baclofen in alcohol dependence that are</span></strong> <strong><span style="color: #888888;">described in Dr. Bucknam’s case study (Bucknam, 2007) and</span></strong> <strong><span style="color: #888888;">in my self-case report (Ameisen, 2005). At a behavioural</span></strong> <strong><span style="color: #888888;">level, alcohol, baclofen and GHB all share sedative/hypnotic</span></strong> <strong><span style="color: #888888;">effects in humans. Clinical trials have shown baclofen to</span></strong> <strong><span style="color: #888888;">reduce anxiety in alcoholic (Krupitsky et al ., 1993; Addolorato</span></strong> <strong><span style="color: #888888;">et al ., 2002) and non-alcoholic (Breslow et al ., 1989;</span></strong> <strong><span style="color: #888888;">Drake et al ., 2003) patients alike. </span></strong></p>
